Historically, The Wealshire provided long-term care for individuals who required assistance with daily activities and had ongoing medical needs. We catered to individuals who were memory impaired or were unable to live independently.

Residents requiring traditional nursing care continue to be an important part of the Wealshire Center of Excellence’s core offerings, including:

  • Chronic Diseases: diabetes, hypertension, heart disease, chronic obstructive pulmonary disease (COPD), severe arthritis and dementia

  • Neurological disorders: Alzheimer’s Disease, Parkinson’s Disease, multiple sclerosis, and stroke care. These conditions often require cognitive support, mobility assistance, and medication management.

  • Respiratory Conditions: Bronchitis & emphysema, which necessitate oxygen therapy, respiratory treatments, and monitoring of respiratory function.

  • Cardiovascular Conditions: Congestive heart failure & coronary artery disease require monitoring of vital signs, medication, and lifestyle changes.

  • Post-surgical and Post-Acute Care: Recovery services and physical therapy for individuals who have undergone joint replacement or major surgery.

  • Terminal Illness: End-of-life care for maintaining comfort

  • Infectious Diseases: Individuals that require isolation or specialized infection control.

Our multidisciplinary group of nurses, therapists, caregivers, and social workers operate as a team-- in coordination with patient’s physicians— to create personalized care plans for each resident.

Our guests includes:

1. Elderly Patients: Nursing care for age-related conditions, chronic illnesses, or functional limitations. Assistance with activities of daily living, medication management, and monitoring for age-related health concerns.

2. Individuals with Chronic Illnesses: Patients with diabetes, heart disease, respiratory diseases, or neurological disorders often require ongoing nursing care. Nurses help manage their conditions, provide education on self-care, and monitor their symptoms and vital signs.

3. Post-Surgical Patients: Nurses monitor their vital signs, manage pain medications, assist with wound care, provide mobility support, and educate about post-operative care and potential complications.

4. Individuals with Disabilities: Patients with physical or intellectual disabilities may need nursing care to support their daily activities, manage their health conditions, administer medications, and ensure their well-being.

5. Palliative and Hospice Care Patients: Specialized care for pain management, symptom control, and emotional support. The focus is on enhancing quality of life and family support.
